The survey includes a series of five activities. Each activity is a little different from the other and helps us to understand your overall experience navigating the Indianapolis food system.

You will begin by filling out your household information before moving onto the first activity. Completing an activity will prompt you to move on the next activity. You will also receive an email with a link to the next activity so that you do not have to take all five activities at once.

*An email is required to complete the activities online.  If you do not have an email you can request a paper copy of the activities.
