HOW TO WIN

We need your input to create the 25-year food plan for Indianapolis. To understand how access to food impacts your life, we ask you to complete a survey that includes five activities. Because your input matters to us, completing all five activities will allow you to win prizes! 

The survey consists of five activities that can be completed at your own pace. For each survey you complete, you will be entered to win a small cash prize. Completing all five activities will enter your name into a  $100 grocery gift card drawing!  

Completing an activity allows you to move on to the next activity. To get started, you will begin by filling out a household survey followed by the first activity. After completing this activity you will receive a follow up email with a link to take the next activity and so on. You may also proceed directly to the next activity by following the link at the end of each survey! Learn more about the activities below.

*An email is required to complete the activities online.  If you do not have an email you can request a paper copy of the activities.

Activity Overview

Household Survey

To begin, we ask that you provide us some brief details about your household.  This information will be super helpful to understanding the community that this plan serves. *Completing this survey is required to start the first activity. All information provided remains strictly confidential.

GETTING GROCERIES

After filling out the household survey, you will begin the first activity. The purpose of this activity is to dig deeper into the experience of getting groceries in Indianapolis.

This section is all about the experience of eating out or ordering take-out in Indianapolis.

This activity is all about healthy food choices and how they fit into your food experience.

PREPARING FOOD AT HOME

This activity is all about your cooking preferences. Do you cook at home? What do you make?

WHAT IF?

Now it’s time to dig deeper with some of the food related challenges you identified and come up with some ideas for improvement!
